Hanging columella correction is a rhinoplasty procedure performed when the tissue between the nostrils extends too far below the alar rims, creating an overly visible or downward-hanging appearance from the profile and frontal view. This condition may be related to the position of the caudal septum, the shape and strength of the medial crura, soft tissue characteristics, or a combination of these factors. The goal of surgery is to improve columellar position while maintaining natural nasal proportions and structural support.
Hanging columella correction before and after photos can help demonstrate how the relationship between the nasal tip, nostrils, and upper lip may change after surgery. Before treatment, the columella may appear excessively exposed, particularly from the side, and the lower part of the nose may look elongated or unbalanced. Some patients may also have associated concerns such as a drooping nasal tip, excessive projection, asymmetry, or an irregular nasal base.
Depending on the anatomy, correction may involve adjusting the caudal septum, repositioning or reshaping the medial crura, refining the nasal tip, or modifying soft tissue support. The surgical approach is tailored to the individual patient, as overcorrection can create an unnatural or retracted columellar appearance.
Swelling is expected after rhinoplasty, especially around the nasal tip and columella. Early postoperative photos may not fully reflect the final result because tissue position continues to settle as swelling decreases. Final refinement may take several months and, in some patients, up to a year or longer.
