Pre-Treatment Planning
Pre-treatment planning is the key to success for open nose surgery. This stage focuses on clarifying the patient’s wishes and expectations. Doctor-patient consultations are important for understanding the patient’s current nasal structure and desired outcome. The doctor evaluates the patient’s nasal measurements and overall facial structure, helping the patient understand what kind of result can be expected after surgery. Computer-assisted simulations may be used to better demonstrate how the nose may look after the procedure. In addition, the patient’s preoperative health condition is assessed, and any existing medical issues are taken into consideration. Pre-treatment planning also includes determining the date of the surgery and providing special instructions that should be followed before the operation. This stage is critical for ensuring patient satisfaction with the surgical results and allowing the procedure to progress smoothly.
Incision in the Columella Area
An important stage of open nose surgery is the incision made in the columella area. This step plays a vital role in the successful completion of the procedure. The columella is the area located beneath the nose that connects the two nostrils. During open rhinoplasty, a W-shaped incision is made in this area. This incision allows the doctor to access the tissues and structures inside the nose comfortably. The columella incision is approximately 2–3 mm in length and is closed with sutures afterward. It must be handled carefully to leave minimal scarring. Although the incision is made in an inconspicuous part of the nose, it is an indispensable step for reshaping and correcting the nose.
Intervention on the Nasal Tissues
During the surgery, the nasal skin is lifted through the incision in the columella, providing better access to the nasal tissues. The doctor works on the cartilage and bone structure of the nose. These tissues are modified to reshape and correct the nose. Nasal curvature is corrected, the nasal tip is reshaped, and interventions are performed to improve breathing function.
Completion of the Procedure
During the completion stage of the surgery, the doctor finalizes the main objectives of the rhinoplasty procedure. At this stage, the structures of the nose are brought to the desired final shape. The cartilage, bones, and other nasal tissues are arranged according to the surgeon’s treatment plan. Nasal curvature is corrected, and the nasal tip is shaped as intended. The doctor makes the nostrils symmetrical and completes the final refinements. The primary goal of rhinoplasty—a more aesthetic and balanced nasal appearance—is achieved. When the procedure is completed, the incision in the columella, which is typical of the open technique, is closed. The sutures used at this stage dissolve on their own. As a result, this part of the surgery is a critical stage that enables the patient to achieve the desired appearance and functional improvements and must be completed with great care.
Duration of the Surgery
The duration of open nose surgery may vary depending on the procedures to be performed and the patient’s individual condition. On average, it takes about 2 hours. More complex cases, such as nasal deviation or breathing problems, may require more time, while simpler procedures may take less time. The operation is performed under general anesthesia, and patients are usually kept under observation afterward.
Recovery Process
The postoperative period is important for patients. Within the first 3–4 hours after surgery, the patient is helped to stand up and encouraged to walk. Heavy sports and intense physical activities should be avoided in particular. Swelling and edema normally occur after surgery, but these usually decrease within one week. Care should be taken to avoid damaging the sutures, and the doctor’s postoperative instructions should be followed carefully.
Open nose surgery is an effective method for nasal reshaping. This procedure can help patients achieve their desired nose shape and breathe more comfortably.
